Our Direct Primary Care (DPC) membership is designed to provide you with comprehensive, unrestricted access to your primary care physician for a flat monthly fee. Your membership covers the majority of your routine healthcare needs without the hassle of insurance billing, copays, or surprise invoices.Standard membership inclusions typically include:Unlimited Office Visits: Whether it’s an acute illness, a routine check-up, or a follow-up, there are no per-visit charges.Extended Appointments: We schedule longer visits (30–60 minutes) so we can actually listen and address all your concerns.Direct Access: Reach your doctor directly via phone, text, or secure email for quick questions and coordination.Preventive Care: Annual wellness exams, school/sports physicals, and health screenings.Chronic Disease Management: Ongoing care for conditions like hypertension, diabetes, and asthma.Basic Procedures: Many in-office procedures—such as simple wound care, cryotherapy, and skin tag removals—are included (though some may require a small fee for materials).

Yes and no—we believe in transparency, so here is how we distinguish between the two:Primary Care Weight Management (Included): Discussing your weight, metabolic health, and lifestyle during your regular appointments is a standard part of primary care and is fully included in your membership. We are happy to monitor your progress, manage weight-related chronic conditions, and offer general nutritional guidance as part of your routine care.Specialized Weight Loss Program (Additional Investment): For patients seeking a more intensive, structured medical weight loss experience, we offer a dedicated Weight Loss Program. This program involves a higher level of frequency, specialized monitoring, and specific resources that fall outside the scope of standard primary care. Because of the intensive nature of this clinical support, this program is available for an additional investment.Note: We recommend starting with a standard consultation to discuss your goals; we can then determine if your needs are covered by your membership or if our specialized program is the right fit for you.

While your membership covers the vast majority of your primary care needs, it is not a substitute for comprehensive health insurance. To keep our monthly fees affordable and our focus on personalized care, certain high-cost or specialized services are handled outside of the membership fee.NOT Covered:Imaging and Advanced Diagnostics: Services such as X-rays, MRIs, CT scans, and ultrasounds are performed at outside facilities. However, we have negotiated "cash-pay" discounts with local imaging centers to save you significant money.Specialist Care: Consultations and treatments from specialists (e.g., Cardiologists, Oncologists, or Surgeons) are not covered.Procedures and Surgeries: Procedures not performed at ShillMed (e.g., Colonoscopy, Outpatient Surgery, Inpatient Surgery, etc.) are not covered.Emergency Room and Hospitalization: If you require emergency department care or an overnight hospital stay, those costs are handled by the hospital and your insurance provider. ShillMed does NOT provide Emergency care. If you have a medical emergency, dial 911.Vaccinations: Due to the high overhead and storage costs, we do not stock vaccines at this time. We can, however, provide prescriptions or direct you to local pharmacies where vaccinations are often covered by insurance.External Lab Work: While the interpretation of labs is included, the actual lab draw, processing fee, etc., from the lab (e.g., LabCorp or Quest) is separate. We offer deeply discounted wholesale pricing for members who choose to pay out-of-pocket. We are also happy to refer you to any lab you prefer. We are also happy to order your labs to be covered by your health insurance.Prescription Medications: Through a partnership with ManifestRx, many common medications are available to you at no charge, including several acute care medications typically prescribed in urgent care. Additional medications can be purchased at your pharmacy of choice either with or without your insurance coverage.Our Specialized Weight Loss Program: While standard primary care weight management is included (discussing weight management a few times each year as part of a typical visit), our intensive, structured Weight Loss Program is available for an additional investment.

Most of our members find that pairing their DPC membership with a High-Deductible Health Plan (HDHP) or a Healthshare provides the best balance of cost and protection. This ensures your day-to-day care is covered by us, while your insurance is there to protect you against the "big stuff" listed above. Transparency is our priority. If a recommended test or treatment falls outside of your membership, we will always discuss the estimated costs with you upfront so there are never any surprises. You can always choose whether to accept our cash pricing or use your insurance coverage!
