Dr. Otto Shill, DO, FACP, DABOM
Overview

Dr. Otto Shill, DO, FACP, DABOM, is a dual board-certified physician specializing in Internal Medicine and Obesity Medicine with extensive advanced training in Lifestyle Medicine. Based in Washington State, Dr. Shill is the co-founder and Chief Medical Officer of ShillMed Weight Loss & Lifestyle Medicine Center. Known for his holistic, concierge-level approach to healthcare, he balances dedicated clinical care with hospital medicine, combining evidence-based medical treatments with individualized wellness planning through an innovative Direct Primary Care (DPC) model.
Medical Education, Training, & Philosophy
Dr. Shill earned his Doctor of Osteopathic Medicine (DO) degree from the Arizona College of Osteopathic Medicine at Midwestern University. He then went on to complete his comprehensive medical residency training in Internal Medicine at MercyOne Des Moines in Iowa.

As an Osteopathic Physician, Dr. Shill’s foundational training emphasizes a whole-person approach to medicine, looking at how the body’s systems interconnect. He incorporates this philosophy via hands-on therapies like Osteopathic Manipulative Therapy (OMT) to optimize musculoskeletal alignment, mobility, and pain management alongside classic Internal Medicine.

With over a decade of clinical experience, Dr. Shill achieved dual board certifications from:

-
The American Board of Internal Medicine
-
The American Board of Obesity Medicine
He is recognized as a Fellow of the American College of Physicians (FACP). Dr. Shill’s overarching philosophy shifts away from standard insurance-driven models toward treating chronic disease, metabolic dysfunction, and everyday health needs at their roots through the pillars of Lifestyle Medicine: nutrition, physical activity and resistance training, restorative sleep, connecting with others, avoiding risky substances, and stress management.

A Personal Journey to Lifestyle Medicine

Dr. Shill’s dedication to metabolic health is deeply personal and rooted in his own transformation. The rigorous demands, poor eating habits driven by high stress, and sedentary nature of medical school and residency took a heavy physical toll, ultimately leading him to a turning point where he had to embark on his own weight loss journey. By diving into the science of behavior change, nutrition, and exercise, he successfully lost 55 pounds—an experience that completely reoriented his clinical path. Driven by his own success, he intensively studied the core pillars of lifestyle medicine, went on to earn his board certification in Obesity Medicine, and fundamentally changed his life.
Through this personal shift, Dr. Shill recognized the profound power of lifestyle medicine as a true approach to proactive healthcare rather than reactive sick care. While he seamlessly utilizes a traditional, evidence-based medical approach where appropriate in his clinic, he places a heavy emphasis on sustainable lifestyle interventions. His goal is to empower patients to reduce the current burden of chronic illness and significantly minimize the risk of developing preventable diseases in the future.

Career & Clinical Practice
Dr. Shill is licensed to practice medicine in both Washington and Idaho, maintaining active medical affiliations to care for patients across multiple settings. His clinical practice spans the full spectrum of adult health, encompassing both hospital-based inpatient Internal Medicine and clinic-based outpatient care. He actively maintains several important certifications, including BLS (Basic Life Support), ACLS (Advanced Cardiac Life Support), and PALS (Pediatric Advanced Life Support) from the American Heart Association (AHA), and ATLS (Advanced Trauma Life Support) from the American College of Surgeons (ACS). This dual experience allows him to manage acute, complex illnesses in hospital environments while seamlessly transitioning patients into long-term preventative care in an office setting.
He runs physical clinic locations in Spokane Valley and Colfax, Washington, while managing a broad remote telehealth practice serving patients across both states.
ShillMed Weight Loss & Lifestyle Medicine Center
Dr. Shill co-founded the clinic alongside his wife, Emily Shill, RN, BSN, who serves as the Chief Nursing Officer. Together, they designed a practice model focused on long-term relationships and personalized, data-driven strategies rather than generic health programs.
At ShillMed, Dr. Shill provides a unique intersection of specialized medicine and ongoing health maintenance:
-
Direct Primary Care (DPC): Operating under a direct-pay membership model, Dr. Shill bypasses insurance constraints to offer patients unprecedented, direct access to their doctor. This includes extended, unhurried visits, highly personalized preventative care, and comprehensive chronic disease management tailored for patients navigating obesity, type 2 diabetes, or severe cardiovascular risks.
-
Medical Weight Management: Safely overseeing customized metabolic and weight loss pathways using clinical tools like advanced body composition scans, nutritional coaching, and structured, medically supervised protocols.
-
Advanced Pharmacotherapy: Carefully monitoring modern medical treatments—including next-generation GLP-1 and GIP receptor agonists (such as semaglutide (Ozempic/Wegovy) and tirzepatide (Mounjaro/Zepbound))—alongside other FDA-approved therapies to navigate metabolic blockages.
-
Hormone & Metabolic Management: Providing perimenopause, menopause, and metabolic hormone optimization to combat modern health concerns like fatigue, chronic inflammation, and sleep apnea.

Personal Life & Hobbies
Dr. Shill resides in the Spokane area with his wife and their five children. Described as a "jack of all trades," he is deeply passionate about learning new technical skills outside the clinic and actively practicing the lifestyle strategies he preaches. He regularly enjoys heavy resistance training, hiking, road biking, and the occasional run, alongside creative and hands-on pursuits like woodworking, cooking, and cultivating heirloom vegetables. He advocates for balance in both life and diet, often noting that sustainable health leaves room for the occasional Oreo or trip to Pete & Belle’s.
