Understanding Diabetes: A Comprehensive Guide for Spokane Patients

As direct primary care and metabolic health providers in Spokane Valley and the broader Inland Northwest, we believe that informed patients are empowered patients.

As primary care providers in the Inland Northwest, we see the impact of diabetes on our community every day. This guide serves as a roadmap for your health journey, moving beyond basic management to focus on cutting-edge metabolic restoration and long-term vitality.

By combining deep clinical expertise with actionable lifestyle strategies, we aim to provide you with the tools to not just live with diabetes, but to actively take control of your metabolic future.

Quick Summary: Metabolic & Diabetes Care at ShillMed

  • The Core Problem: Diabetes is an escalating chronic metabolic crisis in the Inland Northwest, heavily driven by insulin resistance and underlying visceral fat conditions like Lean MASLD.

  • The Shift to Remission: Modern clinical guidelines (ADA/AACE 2026) prove that Type 2 diabetes can achieve sustained remission through aggressive, physician-supervised metabolic weight management.

  • Advanced Local Care: ShillMed provides comprehensive primary care and metabolic restoration in Spokane Valley and Colfax, utilizing advanced onsite diagnostics like InBody Body Composition testing and non-invasive FibroScan technology.

1. Fundamentals: What Causes Diabetes and Metabolic Dysfunction?

What is diabetes, and what causes it?

Diabetes mellitus is a chronic metabolic disorder where the body cannot effectively manage blood sugar (glucose). This occurs because the body either cannot produce enough insulin or cannot effectively use the insulin it makes (StatPearls, 2026).

  • Type 1 Diabetes: An autoimmune condition where the immune system mistakenly destroys the insulin-producing beta cells in the pancreas..

  • Type 2 Diabetes: Accounts for ~90% of cases and is primarily caused by insulin resistance. In 2026, we now recognize "Lean MASLD" as a significant factor, where individuals with a normal BMI can still develop severe insulin resistance due to visceral fat.

2. What Are the Early Symptoms and Diagnostic Criteria for Diabetes?

Diabetes often presents with subtle symptoms that go unnoticed:

  • Vision Changes: High blood sugar can cause the lens of the eye to swell, leading to blurred vision.

  • Slow Healing: Elevated glucose impairs circulation, meaning minor cuts take much longer to heal.

  • Skin Changes: Dark, velvety patches of skin (Acanthosis Nigricans) often appear in skin folds like the neck.

  • Frequent Infections: High sugar environments encourage the growth of yeast and bacteria.

  • Tingling or Numbness: Over time, high glucose damages nerves (Neuropathy), causing "pins and needles" in the hands and feet.

Fasting Plasma Glucose (FPG)
Hemoglobin A1c (HbA1c)
2-Hour Oral Glucose Tolerance Test (OGTT)
Random Plasma Glucose
Diabetes
126 mg/dL or higher
6.5% or higher
200 mg/dL or higher
200 mg/dL or higher (with classic symptoms)*
Prediabetes

5.7% to 6.4%
140 mg/dL to 199 mg/dL
Normal
Less than 100 mg/dL
Less than 5.7%
Less than 140 mg/dL

*Note: In an individual with classic symptoms of hyperglycemia (such as excessive thirst, frequent urination, or unexplained weight loss) or a hyperglycemic crisis, a random plasma glucose level of 200 mg/dL or higher is diagnostic of diabetes and does not require repeat testing.

Why Dr. Shill Looks Beyond Just the Hemoglobin A1c Test

Many clinics rely solely on a standard HbA1c blood draw to screen for diabetes because it doesn't require fasting. However, a major large-scale study presented at the Endocrine Society's annual meeting found that the HbA1c test missed roughly 73% of diabetes cases that were successfully detected by an Oral Glucose Tolerance Test (OGTT). Because the A1c only measures a 3-month average, it frequently overlooks early-stage metabolic dysfunction and post-meal blood sugar spikes. This is why a comprehensive workup—including an OGTT or detailed fasting panels—is vital for true prevention.

Dr. Shill also likes to check a patient's fasting insulin level and HOMA-IR score to better understand your metabolic health early. This allows us to help you implement lifestyle changes to prevent progression to diabetes when insulin resistance begins. Proactive, not reactive.

Note: This 2019 data is heavily supported by broader epidemiological data, such as a large analysis of NHANES data published in the Journal of Clinical Medicine, which similarly showed that the HbA1c test has a standalone sensitivity of only about 34% when compared against the gold-standard OGTT, meaning it misses roughly two-thirds of cases).

Advanced Metabolic Testing in Spokane Valley

Standard blood tests only tell part of the story. Because conditions like Lean MASLD hide dangerous visceral fat deep around your internal organs (even if your body weight or BMI is completely normal), true metabolic assessment requires a deeper look. At ShillMed, we utilize medical-grade InBody Body Composition testing right in our Spokane Valley clinic. This advanced technology goes beyond the scale to map your exact fat distribution and skeletal muscle mass, giving us a precise baseline to track your metabolic restoration.

3. Can Type 2 Diabetes Be Reversed into Remission?

Yes, for Type 2 diabetes. Research like the DiRECT study (updated 2025/2026) has shown that significant weight loss can lead to remission, defined as maintaining an A1c below 6.5% without any diabetes medication for at least three months (Lean et al., 2026).

4. Modern Medical Treatments: What Are GLP-1s, CGMs, and AID Systems?

Current guidelines emphasize treating the whole patient rather than just a number (AACE, 2026).

GLP-1 and Dual GIP/GLP-1 Receptor Agonists

  • Liver Health & Non-Invasive Diagnostics: These advanced agents are a cornerstone in treating MASH (Metabolic Dysfunction-Associated Steatohepatitis) because they drastically reduce liver inflammation. To monitor your liver health without the need for a painful, invasive surgical biopsy, our Spokane Valley office features state-of-the-art FibroScan technology. A FibroScan works like a specialized, painless ultrasound to measure liver stiffness and fat accumulation in real-time, allowing Dr. Shill to precisely track your liver's healing progress as your metabolic health improves.

  • Cardiovascular Protection: They significantly reduce the risk of major cardiac events like heart attacks and strokes.

Diabetes Technology: CGMs and AID Systems

  • Continuous Glucose Monitors (CGM): Devices like the Dexcom G7 and Freestyle Libre provide real-time data to your smartphone.

  • Automated Insulin Delivery (AID): Systems like the Omnipod 5 use algorithms to automatically adjust insulin delivery.

5. Prevention & Lifestyle: Local Spokane Resources

Intensive lifestyle changes can reduce the risk of progressing to Type 2 by up to 58%.

Nutrition & "Deprescribing"

At ShillMed, we focus on whole-food, nutrient-dense patterns. We provide strict medication de-prescribing protocols to safely reduce your doses under close medical supervision as your health improves—preventing dangerous drops in blood sugar. That's right! In many cases, we can help you get OFF medications!

Exercise in the Inland Northwest

  • The Centennial Trail: Perfect for daily brisk 30-minute walks.

  • Riverside State Park: Trails like the Bowl and Pitcher Loop offer manageable hiking.

  • Strength Training: We recommend a minimum of twice-weekly resistance training.
